1. (a) This U Mobile Protector Guard Insurance - Perlindungan Tenang RM75 Voucher Programme (“Programme”) is jointly organised by U Mobile Sdn Bhd (“U Mobile”), U Mobile Services Sdn Bhd (“U Mobile Services”) and Chubb Insurance Malaysia Berhad (“Chubb”). U Mobile and U Mobile Services are collectively referred to as the “U Mobile Group”.
(b) The Programme is an initiative by the Government of Malaysia to offer incentive vouchers worth RM75 each to eligible recipients of Bantuan Keluarga Malaysia (“BKM”) for the purchase of Perlindungan Tenang Insurance product.
2. This Programme will commence at 08:00 on 20 January 2022 and will end at 23:59 on 31 December 2022 (“Programme Period”).
3. By participating in this Programme, you agree to be bound by all the terms and conditions below (“Terms and Conditions”).
4. Programme Eligibility
a. This Programme is open to selected active U Mobile Prepaid or Postpaid subscribers (able to make and receive calls via U Mobile Prepaid or Postpaid services) who must fulfill the following requirements:
(i) Malaysian citizens;
(ii) BKM recipients;
(iii) meet the Perlindungan Tenang eligibility as verified via by https://www.myptv.my;
(iv) have applied for Chubb’s personal accident insurance (“Protector Guard Insurance”); and
(v) between the ages of sixteen (16) and sixty-four (64) years old upon registering for the Protector Guard Insurance.
b. U Mobile Group and Chubb reserve the right to reject any application by any eligible subscriber to subscribe to the Protector Guard Insurance without assigning any reason.
c. Eligible subscribers who are below the age of 18 years old are deemed to have obtained the consent of their parents or guardians to participate in this Programme.
5. Programme Mechanics
a. To participate in this Programme, you must opt-in to subscribe to the Protector Guard Insurance during the Programme Period via SMS or MyUMobile App (“Opt-In”) when you are notified of your eligibility.
b. You will receive a notification via SMS from U Mobile once you have successfully opted-in (“Opt-In Notification”).
c. The Protector Guard Insurance is underwritten by Chubb subject to the applicable terms and conditions imposed by Chubb which are available at https://www.chubb.com/content/dam/chubb-sites/chubb-com/my-en/campaign/protector-guard/Perlindungan%20Tenang_Protector%20Guard_PDS%20(18%20months).pdf U Mobile Group makes no representations nor warranties on the Protector Guard Insurance.
d. Each eligible subscriber is only entitled to Opt-In once during the Programme Period.
6. RM75 Incentive Voucher for Protector Guard Insurance
a. The eligible subscriber who fulfils the requirements of paragraphs 4 and 5 above shall receive an incentive voucher worth RM75 (“Incentive Voucher”). The Incentive Voucher is subject to availability and is managed by Insurance Services of Malaysia. You are entitled to receive the Incentive Voucher only once during the Programme Period.
b. The Incentive Voucher shall be automatically used to pay the Protector Guard Insurance premium for a period of 18 months from the date of the receipt of the Protector Guard Insurance e-certificate by the eligible subscriber from Chubb. No payment will be required to be made by that eligible subscriber to Chubb during this 18 month Protector Guard Insurance coverage period.
c. In addition to the Incentive Voucher, eligible subscriber will also receive bonus credit worth RM5 each (“Additional Bonus Credit”) during the Programme Period.
d. The Additional Bonus Credit will be credited to the eligible subscriber’s U Mobile Postpaid or U Mobile Prepaid account (as applicable) within 30 working days from the date of the eligible subscriber’s Opt-in.
e. Each eligible subscriber must ensure that their U Mobile Postpaid or U Mobile Prepaid account is active at the time the Additional Bonus Credit is credited into their account (able to make and receive calls via U Mobile Prepaid or Postpaid services).
a. U Mobile Group or Chubb may change any of these Terms and Conditions and may terminate, suspend this Programme or extend the Programme Period. Any such change will be announced on our website or the website of Chubb. It is your responsibility to check these Terms and Conditions on our website and the website of Chubb and if you do not agree to any of the changes, you must immediately cease participation in the Programme.
b. By participating in the Programme, all eligible subscribers: -
(i) consent for U Mobile Group to collect, record, hold, store, use and disclose their personal information for purposes which are necessary or related to the participation in the Programme;
(ii) consent for U Mobile Group to disclose or publish their personal information such as their names and identities and any general information that U Mobile Group sees fit about the participants in any media, marketing or advertising materials;
(iii) grant U Mobile Group the absolute and unrestricted right to modify, use and publish any still or moving images of the participants for any promotional, marketing, commercial or other related purpose, without any payment or compensation; and
(iv) agree that their personal information shall be used by Chubb in accordance with Chubb’s Personal Data Protection Notice at https://www.chubb.com/my-en/public-information-disclosure/personal-data-protection-notice.html.
c. To the extent permitted by law, the U Mobile Group excludes all liability in connection with this Programme. All eligible subscribers and if they are below 18 years of age, their parents or guardians, agree to release the U Mobile Group from and indemnify the U Mobile Group against all liability (including negligence) for any personal injury, for loss or damage (including loss of opportunity) or breach of any of these Terms and Conditions; whether direct or indirect, special or consequential arising out of participation in this Programme.
d. The Programme and these Terms and Conditions are governed by the laws of Malaysia. The eligible subscriber agree that the Courts of Malaysia have jurisdiction over all matters arising from this Programme.
